Use form CG-5667, attached. For Unit, enter RB-M hull number and the words "Preliminary Acceptance Trials". The blocks for Serial Number, Conducted By, and Approved By may be used at the Contractor's discretion. In the block labeled Standard Conditions During Operations, document any unusual results (including initial unsatisfactory results and the steps taken to bring the system into compliance), any deviations from the approved test plan (CDRL 441-001), and certify the VHF-DF system to be in compliance with contract requirements. If additional space is required to document this information, state the number of continuation page(s) and attach Contractor-formatted continuation pages as needed.
